Primary Health Care Hub Smithton
Overview
The Primary Health Care Quality and Coordination program is an Australian Government initiative administered by the department, under funding Outcome 1, Program 1.6 Primary Health Care Quality and Coordination. The program aims to deliver funding to frontline primary health care services and improve access, delivery, quality and coordination of those services. Key priorities include improving health outcomes, especially for those with chronic and mental health conditions, and reduce unnecessary hospital visits. Individual grants under this program may have a designated use period (DUP) applied. The purpose of this grant opportunity is to ensure that residents of Tasmania’s far north-west have access to the healthcare they need. The Smithton Health Hub will be a modern, purpose-built facility, centrally located in Smithton, designed to meet the growing healthcare demands of the Circular Head region.
